Bus Pass.pdf Blue bus pass form must be filled out completely and signed by the parent/guardian. This form should be submitted to the office on the day of the change. Per School Committee ruling, kindergarten students are not permitted bus passes unless an emergency situation arises. Such a change must be authorized by the principal. Pick-up Note.pdf Yellow pickup slips must be used for after school pick-up as well as notification of early dismissal. PLEASE NOTE: Dismissal changes cannot be taken over the phone. In the case of an emergency, a FAX including a parent/guardian signature may be sent to request a dismissal change. A telephone call to school must either precede or follow the fax. Office Fax Number 978-266-2573 Lunch Tickets.pdf Prepaid lunch, milk and ice cream tickets can be purchased on Monday,Tuesday, Wednesday or Friday between 8:00 a.m. and 10:00 a.m. from the cafeteria manager. Tickets must be purchased in strips of five and may be used any time during the school year. Pricing: 5 lunch tickets (milk included) for $10.00 5 milk tickets for $2.00 5 ice cream tickets for $2.50 To purchase tickets, please send cash or check.
